Hi Mike,
CW is most definitely used for EME above 432MHz. Although JT65C is very effective on 1.3 and 2.3GHz, CW is still the preferred mode. This is mainly due to a very strong pro CW attitude amongst many of the EME operators. Having said that, the level of JT65C on 1.3GHz is VERY high relative to the number of operators on EME overall. As you have probably seen, quite small EME systems are capable of making successful QSOs even with low ERP using this mode.
JT4G also works on EME. However, the poorer sensitivity of this mode (compared to JT65C) limits its use to the higher bands where libration can render JT65C unusable. The digital 'cw' mode developed for the 47GHz EME QSOs is a bit different again and doesn't seem to have caught on to any great extent. I can't remember its designation!
Hellschrieber has also been used in EME.
